Fees and Billing Arrangements
All services performed at this practice are bulk billed to patients upon the presentation of a Medicare card. A list of fees for non-Medicare card holders is shown in the reception area.
Personal Details
It is our practice policy to update your Medicare Card, Healthcare/Pension Card (if applicable) and personal details at every visit. This allows us to keep your most current details should we need to contact you in emergencies.
Appointments
Please ring (02)9745 6728 for an appointment. Every effort will be made to accommodate your preferred time and GP. Emergencies will always be given priority and our reception staff will attempt to contact you if there is any unforeseen delay or your GP has been called away. Longer consultation times are available, so please ask our receptionist if you require some extra time. If you or a family member requires an interpreter service, we can organise this for you. Please let us know when you make the appointment.
Longer Consultations are available on request. Please let reception staff know when making your appointment or when you arrive at reception that you require a longer consultation.
Missed or Cancelled Appointments
If your not able to attend your appointment you need to give us at least 24 hours notice so that the appointment time can be offered to another patient..
Privacy and Confidentiality
This practice adheres to the provisions under the Privacy Act, therefore it is our policy to maintain security of personal health information at all times and to ensure that this information is only available to authorised personnel. If you would like further information about our privacy procedure and release of information, please talk to your doctor or the receptionist.

Reminder System
Our practice is committed to providing you with the best preventative care possible. Practice staff will seek your permission to be included on our reminder system and may issue you with a reminder notice on occasion. If you do not wish to be a part of this system, please let your doctor or practice staff know.
Management of Your Personal Health Information
Your personal health information is collected and used directly in association with your health care. It is a confidential document and it is the policy of this practice to maintain the security of medical records at all times. We will ensure that your information is only available to authorised staff or seek your permission before disclosing it to any third party.
If you have any concerns about your health information privacy, please let your doctor or reception staff know.
Cultural Background and Ethnicity
It is the commitment of our practice to provide the best preventative care. To ensure that your care is tailored to your needs, staff at this practice encourage patients to identify their cultural background and/or ethnicity on their medical record. If you identify with a particular cultural background, please let you doctor or reception staff know.
Telephone Access
Incoming telephone calls to the doctors are restricted to matters of medical urgency. GPs in the practice may be contacted during normal surgery hours. If the GP is with a patient, a message will be taken your call will be returned as soon as is practicable. Your call will always be put through to the GP in an emergency.
After Hours Or Home Visits
If you are unwell and unable to attend the practice during our opening hours and would like a home visit please contact Sydney Medical Service Home GP our after hour service on (02) 8724 6300
Test Results
All incoming pathology and radiology results and specialists letters are reviewed by Doctors. Please make an appointment to obtain results and to plan further follow-up care. Our reception staff cannot provide patients with medical results as all test results are strictly confidential and in order to protect your privacy, this practice does not give results to patients over the telephone.
This practice has a No Smoking Policy.
Patient Rights
If you have a problem with your health care or have had a negative experience when visiting our surgery, we would like to know.
Please feel free to discuss any problems you may have with your doctor or the practice manager. Alternatively you may want write to us or use one of our complaint feedback forms kept at reception.
All complaints are taken seriously and you can remain anonymous if you wish.
